fix(cli): ship a single hexclave bin so pnpx/pnpm dlx can resolve it

`@hexclave/cli` exposed two bins (`hexclave` and `stack`), neither matching
the package's unscoped name (`cli`). When a package has multiple bins and
none matches the unscoped name, `npx` silently runs the first bin, but
`pnpm dlx`/`pnpx` refuses to guess and fails with ERR_PNPM_DLX_MULTIPLE_BINS
— so `pnpx @hexclave/cli <cmd>` was broken while `npx` worked by luck.

Drop the `stack` bin, leaving a single `hexclave` bin that both npx and
pnpm dlx auto-resolve. Follow-through:
- resolveBinName now prefers `hexclave` for the npx self-update re-exec, so
  it targets a bin guaranteed to exist in @latest.
- program name and user-facing `stack <cmd>` guidance strings → `hexclave`.
- tests updated for the new bin preference.
